Case Management Service
The Case Management Service provides individualised support to school leavers with moderate to very high support needs. The service targets individuals with a primary physical disability. Training and support programs are based on the individual needs of each person. These programs aim at further developing the skills required for enhanced community integration and access to employment.
You are eligible if:

You leave school the year the program is being offered. |

You are 18 years old or over. |
|
You are unlikely to be eligible for other employment or vocational options in the short term. |
|
You have moderate to very high support needs. |
|
You are funded under the Post School Options or Community Participation Program. |
What Can We Offer You?
Our programs are designed to suit the needs and goals of the individual.
As a consumer of the service you can receive both 1:1 support with staff or group activities, allowing for personal and effective service provision. Some of the areas in which programs are offered include transport and access; links with education and training services; social activities and community integration; recreational activities; resources for resume development and job searching.
Individual Plans
An individual Plan is written for each person to ensure that their needs are met and their goals are achieved in an accountable and personalised way. The Individual Plan will outline each goal, how it will be achieved and who is responsible for it. In this way, you know that you will receive the type of service you want.
